In this communication we describe 11 cases of unilateral mammary Paget's disease, in which an unequivocal cytodiagnosis was made from nipple scrapings. All 37 women with a palpable mass had a pathological mammogram, 36 of them had carcinoma with an invasive component, 35 (95%) a cancer at a distance from the nipple, 31 (84%) a multifocal carcinoma. All 11 patients with Paget's disease and an associated lump proved to have invasive ductal carcinoma; five also had associated positive axillary nodes. Archival tumor tissues from 14 patients diagnosed between 1990 and 1999 with Paget disease of the breast were analyzed for these molecular markers by using an automated immunohistochemical assay. The majority of patients (97%) presented without an associated palpable mass. The treatment of choice is shown to be radical or modified radical mastectomy with adjuvant radiotherapy. An underlying carcinoma was found in 60 cases (98.4%), atypical epithelial hyperplasia in one. All 11 cases were subsequently proven histologically to be Paget's disease. Average tumor size was 1.6 cm (range, 0.2-3.5 cm). In five of these cases an underlying ductal carcinoma of the breast was simultaneously diagnosed by fine needle aspiration (FNA) cytology of a palpable breast lump. A change in sensation of the nipple-areola, such as itching and burning, is a common presenting symptom. Definitive treatment consisted of modified radical mastectomy in 32 patients, radiotherapy only in 2, and one patient had no definitive treatment.
